The Ministry of Health has requested a Vaccination Coverage Certificate to be submitted by every Grade 1, Year 1, Year 4 and new student. Please print and take the attached certificate to the Vaccination Centre at the old hospital, together with a formal record of your child’s vaccinations, to be stamped. Alternatively, you can have the vaccination certificate signed and stamped by your child’s doctor.
Please return the Vaccination Coverage Certificate to the front office by Friday the 20th of November.
